Our system was upgraded beginning September 1, 2017. Prior to that date, we asked customers to register their warranty using this form. After September 1, 2017, we registered your warranty automatically at the time of purchase.
If you purchased directly from a Daiwa Salesperson after 9/1/17, your warranty was registered at the time of purchase. You do not need to register. However, if you purchased from a third-party seller, you may use this form to register your chair’s warranty, which is necessary if you want to make a repair request.
The manufacturer warranty covers one year labor and two years parts. If you would like to purchase an extended warranty, please click here
